Help Gauge the Health of Your Community

Helping a community reach its full potential takes collaboration.  Not only between community partners such as public health, hospitals, health centers and other important organizations, but the people must be involved as well.  No one has a better understanding of what they need than the people themselves.  That is why the public is being asked for their help in a very important survey. 

October 23, 2012 a Community Needs Assessment survey will be mailed to randomly selected households of 15 counties and three Indian Reservations in North Central Montana.  The goal of the survey is to help assess the health needs of each of these respective communities.  The information that will be gathered can help guide public health jurisdictions, local hospitals, community health centers, and other vital community partners in their efforts to improve the health of the communities they serve

The Toole County Health Department hopes that residents of the communities within the region will see this as an opportunity to tell us how we can support them with their concerns specific to the health of their community as a whole.  The information that we are asking for is vital and will be used to develop a specific  Community Health Improvement Plan for Toole County, along with gaining a regional perspective of what is needed to attain and/or sustain a healthy community and enhance the quality of life for all residents!

Recipients of the survey are asked to respond; surveys should take ten minutes or less to fill out. Those who complete and return their survey by December 4^th will be entered in a drawing for a chance to win a $100 dollar gift card.
